Forefoot overload with pain under the metatarsal heads, whether from fat-pad atrophy, tight calves, or simply miles. The template shifts load off the heads and onto structures built to carry it.

The forefoot platform is where this template does its work, so the device needs to extend under it. Pads and reliefs are placed against the scan and printed as one piece on the flat base, so they cannot migrate the way stick-on pads do.
